As the coronavirus, or COVID-19, spreads globally and cases rise in the US, some Ho-Chunk Nation events, classes, facilities, etc., will be postponed, closed, or canceled out of an abundance of caution.

ID documents refers to:
Change of Address form ~ driver’s license ~state ID ~ passport ~ expired tribal ID ~ Certified Birth Certificate (Minor)
Name Change documents refers to:
Request for Information form ~ Certified Marriage Certificate ~ Copy of Divorce Document ~ Copy of Social Security Card with new name
Tribal ID Fees
*Temporary cards as issued when an ID card is expired and the Member is unable to report to the Office for renewal. A temporary ID card will expire in six (6) months. When Temporary ID cards are renewed, there are no additional charges.
A face mask is required before and after taking your picture. Tribal members must be symptom free.
If you are experiencing COVID-19 symptoms or were exposed, please cancel and reschedule your appointment as soon as possible. Cancellation and Rescheduling can be done by following the link on your confirmation email.
Minor children must be accompanied by an Adult.
